information needed below is used to create your Certificate of Origin for
registration at the DMV. Please make sure you use the Vin number stamped
on your frame. This is located below your glove compartment underneath a
plastic snap in cover. If you purchased more than one scooter, please submit
a separate form for each scooter. If you are a dealer, please fill in your
company name to put on the MCO. Average turn around time is 1 week.
